Meron ka bang Unclaimed Benefit o Unidentified Deposit sa UPPF?
Please check if your name is on the following lists. If so, please file the appropriate claim at a UP Provident Fund office near you. The first list contains names of former members (separated, inactive and charter) with equities that still remain unclaimed from the Fund as of January 17, 2024. The second list shows deposits made by unidentified members (these members failed to present validated deposit slips to the Fund).
If the amounts amounting to P500 and below remained unclaimed three (3) years after this publication, the amounts shall be forfeited in favor of UPPF, as approved by the UPPF Board of Trustees during its 04 December 2023 meeting. Please be reminded that for unclaimed equities, ongoing maintenance and dormancy fees of P500 are charged monthly. Maraming salamat po!
Former UPPF members (Separated, Inactive and Charter) with Unclaimed Equities as of January 17, 2024
For Separated members, please file a claim by submitting: (a) a signed letter request to claim the remaining equity; (b) UP service record; (c) UP clearance; (d) photocopy of two (2) valid governmentissued IDs; (e) bank account number and (e) if applicable, authorization letter or Special Power of Attorney granting authority to a proxy and photocopy of 2 valid government-issued IDs of the proxy. Please claim your equity immediately as maintenance and dormancy fees of P500 per month apply.
For Inactive and Charter members who are currently employed in the University, you may reapply for membership. For those already separated from the University, you may claim your remaining equities subject to the policies implemented by UPPF.

The following deposits were received by UPPF but not credited to any member. Please submit a copy of the validated deposit slip or any proof of payment if you previously made the following bank deposit during said dates.
